Project Reports

Grant recipients are expected to make annual reports on their projects until the funds are spent in full. Please check your Grant Agreement for any special requirements.

Note: Documents must be submitted as Word (97-2003), Excel (97-2003) or a PDF document.

To submit a Project Report:

1. Download the Project Report Form.

2. Complete the form, saving it in your own files.

3. Sign in, using the username and password you used in submitting the project application.

4. On the "Organization Log-In" page, look for the link to Upload Report and follow the instructions from there.
